A breast augmentation – or augmentation mammoplasty to employ the technical term – is a surgical procedure that uses implants or fat harvested from the patient’s own body to improve the size and shape of a woman’s breasts.
The introduction of a silicone breast implant, associated with injection of fat (from the patient’s body), is currently the technique that gives the most natural results: it is called combined mammary augmentation. This gives a much improved cosmetic result and the implants cannot be detected. The implants are placed before the pectoralis muscle (which is the natural position of the mammary gland) and during the same operation, the injected fat is evenly spread (nappage) between the implant and skin. This procedure is particularly suitable for slim women because this nappage of fat perfectly conceals the implants. Moreover, fat injections improve the skin quality thanks to the regenerative effects of the stem cells that are naturally present in our fat (biostimulation).

WHAT IS THE AIM OF BREAST AUGMENTATION?
This intervention will give volume to the bust thanks to the placement behind the breast of implants filled with a gel. In some cases, the implant is not needed and can be replaced by fat harvested from the patient’s own body (thighs, bottom, belly…).
When implants are used, Dr Franchi is among the surgeons who have chosen the revolutionary American innovation called «Keller Funnel®» that allows inserting the implants by reducing the risk of complications. This procedure allows introducing the implant without risk of damage. The risk of infection and of capsular contraction is also reduced. The skin is not traumatized and the final scar is therefore nicer.
WHICH KIND OF EXAMS ARE REQUIRED BEFORE THE INTERVENTION?
A pre-surgery mammography is obligatory. A blood test will be performed to assess your blood coagulation.
WILL I HAVE A SCAR?
Yes, there will always be a scar; however, everything is done to make it, if possible, not visible: placed in the areola, hidden in the fold under the breast or under the arm. Its position will be chosen during the consultation, after discussion with Dr Franchi.
WHAT KIND OF ANAESTHESIA WILL BE USED FOR A BREAST AUGMENTATION?
Most often general anaesthesia, but in some cases it could be a local anaesthetic infusion.
HOW LONG WILL SURGERY TAKE?
About 1 hour.
HOW LONG DO I HAVE TO STAY AT THE CLINIC?
From few hours (ambulatory surgery) to 24 hours (admission in the morning of the intervention and departure the morning after) according to the type of surgery.
WHEN CAN I GO BACK TO WORK?
After three to ten days. However, you recovery time depends on the type of intervention and the natural healing capacity of your body.
WILL I FEEL PAIN AFTER THE OPERATION?
Pain is normally moderated and controlled by painkillers.
WHEN CAN I TAKE A SHOWER AFTER A BREAST AUGMENTATION WITH IMPLANTS?
A shower is recommended already on the morning after the intervention.
DO I HAVE TO WEAR A BRA AFTER THE OPERATION?
No bra should be worn during the first month and then all bra types are allowed.
MUST I STOP DOING SPORT?
Stopping all physical activities involving the upper part of the body is recommended for one month. Similarly, heavy weights should not be lifted for the same amount of time. «If it is painful, I do not do it».
DO I HAVE TO MASSAGE MY BREASTS OR PUT CREAMS AFTER THE OPERATION?
A simple hydrating cream is recommended for few days after surgery.
CAN I TRUST IMPLANTS MADE OF SILICONE GEL?
All implants proposed by Dr Franchi are high-quality and last-generation implants. Most implants carry a life-long guarantee by the manufacturer. In the unlikely case of implant rupture, new implants will be given free of charge. You will receive a guarantee card that will give you the right to a life-long insurance for your implants.
DO I HAVE TO STOP SMOKING BEFORE AND AFTER THE INTERVENTION?
Smoking can increase the risk of infection of the respiratory tract and can slow down the healing process. Smoking can also increase the risk of infection in the area concerned by surgery. If you smoke, we will ask you to reduce or stop smoking in the weeks before and after the intervention.

A LIFE-LONG FOLLOW-UP

HOW TO CHOOSE YOUR BREAST IMPLANTS?
In France, three types of implants are available:
- implants filled with silicone gel
- implants filled with physiological saline solution
- implants filled with hydrogel (carboxy-methyl-cellulose)
Implants filled with silicone gel (1.) give the most natural results. The new generations of silicone gel have a thicker consistence and maintain the shape in the case of implant envelope rupture (the gel does not leak into the body).
The new-generation implants offer a large range of sizes and shapes.

WHAT ARE THE LONG-TERM CONSEQUENCES OF BREAST AUGMENTATION?
Breast augmentation is a long-term commitment and a new surgical intervention may be required later, after the first intervention. It is currently recommended to change implants every ten years. You must be aware that a new intervention to change your breast implants may be needed in the future.
