Lipofilling, lipostructure, fat injection

The injection of autologous fat (harvested from the patient) is a natural procedure that allows restoring the lost volumes, on the face (such as cheeks or chin) or the body (breast, bottom…).

This procedure can be performed on its own or in association with another technique. For instance, fat injection in the face during a face lift improves the final result and gives back a youthful appearance. In the breast, fat injection is often associated with the insertion of implants with the aim of avoiding imperfections, particularly in slim women (this technique is called «combined»).

WHY IS THE INJECTION OF FAT (“LIPOFILLING”) IMPORTANT?

We know that skin damaged by the sun is regenerated after fat injection, with an increase of its collagen and elastin.

For this reason, lipofilling is a very interesting procedure for our patients. It is performed during almost all face lifts to restore the volumes lost with age and to improve the skin texture.

These are the main benefits. As the injected fat cells belong to the patient (“autologous” fat), there is no rejection or reaction towards a foreign body; the body recognizes and accepts its own cells. There are very few complications. The injected fat cells produce molecules that rejuvenate the skin and facilitate the healing process (stem cells called “Adipose-Derived Stem Cells”).

Face and neck lift surgery

More and more women and men are worried about the appearance of their face. Skin loosening is considered as a sign of ageing. Lift surgery allows rejuvenating the entire or part of the face. The result must be perfectly natural. The "healthy glow" effect and the rested face appearance after this procedure increase the self-esteem.

The face oval (lower part of the cheeks) is the first region that loses its elasticity and sags with time. A face and neck lift corrects this by tightening the skin and by eliminating the excess fat (jowls, double chin). It also restores to the face some lost fullness that gives a more youthful and rested appearance. Modern face lift surgery gives a perfectly natural aspect, without any appearance of tight or pulled face.

Moreover, the simultaneous use of biostimulation with platelet-rich plasma (PRP) improves the skin quality and slows down the appearance of new wrinkles in a particularly efficient manner.

Several techniques – sometimes in association – can be proposed on the basis of the identified problem and of the patient’s needs. Your plastic surgeon will carry out a complete exam to determine the best option for you.

Practically, a good result must be natural, without any evident sign of surgery. Lower face and neck lift are very common procedures that can improve the appearance of a women or a man and can contribute to developing his/her self-esteem. Most patients say that the intervention had a very positive effect on their life.

FREQUENT QUESTIONS (FACE LIFT)

Please, remember that only a consultation with a plastic surgeon can give you personalized information.

WHICH KIND OF EXAMS ARE REQUIRED BEFORE THE INTERVENTION?

A blood test will be performed to assess your blood coagulation. A standard heart exam is recommended.

WHAT HAPPENS DURING THE INTERVENTION?

Your surgeon will not cut or shave any hair for this intervention.

The first step consists in injecting under the face and neck skin a solution that limits bleeding, reduces the risk of haematoma and pushes deep down the small nerve terminations to protect them.

The scar is always very discreet (in the ear and then it extends for few centimetres in the scalp). A recent technique does not give any visible scar behind the ear, thus allowing women to have any hairstyle type without risk of seeing the scar (technique described by Daniel Marchac in 2002). In conclusion, scars are always very inconspicuous and difficult to detect, including by your hairdresser.

The loosened neck muscles are tightened. The skin is pulled back and the excess is removed. Frequently, the cheek volume is slightly increased in the case of hollowness, as well as the chin volume, if it is recessed (normally by injection of the patient’s own fat harvested under the chin or in the body).

A head bandage is applied around the face and neck for the first night (eyes, nose and mouth are not covered). It is removed the morning after and the head is washed by a nurse helped by a specialized hairdresser.

The patient can leave the clinic the morning after the intervention, without any bandage.

WHAT KIND OF ANAESTHESIA WILL BE USED?

Most of the time local anaesthetic infusion is used; however, it can be general anaesthesia.

HOW LONG WILL THE PROCEDURE TAKE?

Between 1.5 and 2.5 hours, depending on the type of lift surgery.

HOW LONG DO I HAVE TO STAY AT THE CLINIC?

Ambulatory intervention or overnight.

WHEN CAN I GO BACK TO WORK?

One to two weeks after the intervention.

However, your recovery time depends on the natural healing capacity of your body. Normally, family and friends will not detect any sign of a recent operation, but will notice that the face has a «healthy glow» or a «rested appearance».

WILL I FEEL PAIN AFTER THE INTERVENTION?

Pain is usually moderate and can be controlled by painkillers. During the intervention, your surgeon will inject a local anaesthetic in the areas directly affected by the surgical procedure to desensitize them for several hours after surgery.

WHEN CAN I FIRST WASH MY HAIR AFTER FACE AND NECK LIFT SURGERY?

A bandage will cover your head and face during the first night and will be removed the morning after. Hair will be washed the morning after the intervention.

MUST I STOP DOING SPORTS?

Physical activities should be avoided for four weeks.

DO I HAVE TO MASSAGE THE TREATED AREAS OR PUT CREAMS AFTER THE INTERVENTION?

A healing cream is prescribed for the scars.

DO I HAVE TO STOP SMOKING BEFORE AND AFTER FACE AND NECK LIFT SURGERY?

Smoking can increase the risk of infection of the respiratory tract and can slow down the healing process.  Smoking can also increase the risk of infection and necrosis of the areas directly affected by the surgical procedure. If you smoke, we will ask you to reduce or stop smoking for one month before and one month after the intervention.

Rhinoplasty

«My nose is too long»
«I do not like my profile»
«I would like to reduce the hump of my nose»
«My nostrils are too large and the tip is too bulbous»

Nose surgery is a very common procedure that can improve the appearance of women and men and contribute to developing their self-esteem. Most patients say that the intervention had a positive effect on their life.

Rhinoplasty allows remodelling the nose to give it a more harmonious shape. In most cases, the nose size is reduced by thinning its tip and by eliminating visible humps on the bridge. Sometimes, it is necessary to reduce the nostril size.

Several techniques – sometimes associated – can be proposed in function of the identified problem and the patient’s aspirations. Your plastic surgeon will carry out a detailed assessment of your nose and your face to determine the better option for you. Practically, a good result must be natural, without obvious signs of surgery and in harmony with the rest of the face.

Rhinoplasty complications are rare, but in about 5% of patients, a new touch-up intervention is required to correct minor irregularities or an asymmetry.

If you are thinking about undergoing surgery on your nose, please read Dr Franchi’s information on cosmetic surgery for the nose (rhinoplasty).

FREQUENT QUESTIONS (RHINOPLASTY)

Please, remember that only a consultation with a plastic surgeon can give you personalized information.

WHICH KIND OF EXAMS ARE REQUIRED BEFORE THE INTERVENTION?

A blood test will be performed to assess your blood coagulation. In specific cases, a specialized ENT («ear-nose-throat») examination is required to study the quality of the nose mucosae and the appearance of the nasal septum.

WILL I HAVE A SCAR?

In most cases, there will be no visible scar because the incision will be done inside the nose.

WHAT KIND OF ANAESTHESIA WILL BE USED FOR A RHINOPLASTY?

Most often general anaesthesia, but it may be local anaesthetic infusion when the nose bone is not touched.

HOW LONG WILL SURGERY TAKE?

Between 1 and 2 hours, depending on the importance of the operation.

HOW LONG DO I HAVE TO STAY AT THE CLINIC?

For 24 hours in the case of complete nose surgery.

A few hours in the case of nose tip correction.

WHEN CAN I GO BACK TO WORK AFTER A RHINOPLASTY?

Five to ten days after surgery. However, you recovery time depends on the natural healing capacity of your body.

WILL I FEEL PAIN AFTER SURGERY?

Nose is usually not painful after this intervention.

MUST I STOP DOING SPORT?

All physical activities should be avoided for two weeks after surgery. Contact sports should be stopped for two months after the intervention.

DO I HAVE TO STOP SMOKING BEFORE AND AFTER THE INTERVENTION?

Smoking can increase the risk of infection of the respiratory tract and can slow down the healing process.  Smoking can also increase the risk of infection of the areas directly affected by the surgical procedure. If you smoke, we will ask you to reduce or stop smoking for one month before and one month after the intervention.

Eyelid surgery

«I look always tired, although I am very fit»
«I have bags under my eyes»
«My sagging eyelids bother me when I put on make-up»

Eyelid surgery is a simple intervention that brings immediate and extremely rewarding results for the patients.

It gives a brighter, more open look and eliminates the tired and sad-looking appearance about which patients frequently complain. The technical progress in the last years allows obtaining a more natural result with fewer complications. This intervention may concern only the upper eyelids, only the lower eyelids or the four eyelids.

Dark circles and hollowness under the eyes give a sad-looking and tired face, even when one feels perfectly well. This can be reduced by micro-lipofilling, a procedure whereby fat micro-particles are injected to give back volume to the areas that appear hollow. This technique, called under-eye lipofilling, can be used also to fill in facial wrinkles, hollows and lines. Moreover, fat injections improve the skin quality through the regenerative effect of stem cells that are naturally present in our fat (biostimulation).

Eyelid surgery (called blepharoplasty) is perfect to give to your eyes a fresh and radiant appearance!

FREQUENT QUESTIONS (UPPER EYELIDS)

Please, remember that only a consultation with a plastic surgeon can give you personalized information.

This intervention removes the surplus skin and protruding fat bag that give to the eyelids a «heavy», «thick» appearance. This simple intervention is often enough for correcting the tired eye expression. Several techniques – sometimes associated – can be proposed in function of the identified problem and the patient’s aspirations. Your plastic surgeon will carry out a complete assessment to determine the better option for you. Practically, a good result must be natural, without obvious signs of surgery.

WHICH KIND OF EXAMS ARE REQUIRED BEFORE THE INTERVENTION?

A blood test will be performed to assess your blood coagulation. A standard eye examination is carried out before this intervention.

WILL I HAVE A SCAR?

The incision is done in such a manner as to place the scar in the natural fold of the eyelid, thus making this scar invisible.

WHAT KIND OF ANAESTHESIA IS REQUIRED?

Most often local anaesthesia will be used with or without infusion. It may be general.

HOW LONG WILL THE PROCEDURE TAKE?

About 30 minutes for the two upper eyelids.

About 1 hour for the four eyelids.

HOW LONG DO I HAVE TO STAY AT THE CLINIC?

Few hours (ambulatory intervention: admission in the morning and departure a few hours after the intervention).

WHEN CAN I GO BACK TO WORK?

Six to eight days after the intervention. However, you recovery time depends on the natural healing capacity of your body.

WILL I FEEL PAIN AFTER SURGERY OF THE UPPER EYELIDS?

Normally there is no pain after this intervention.

MUST I STOP DOING SPORT?

Physical activities should be avoided for two weeks.

DO I HAVE TO STOP SMOKING BEFORE AND AFTER THE INTERVENTION?

Smoking can increase the risk of infection of the respiratory tract and can slow down the healing process. If you smoke, we will ask you to reduce or stop smoking in the weeks before and after the operation.

FREQUENT QUESTIONS (LOWER EYELIDS)

Please, remember that only a consultation with a plastic surgeon can give you personalized information.

As times goes by, fat pockets can develop in the area of the lower eyelids that make your eyes look baggy and tired. In most patients, surplus skin in the lower eyelid is minimal and does not need to be removed; only the protruding fat will be eliminated. A little incision hidden in the inner part (mucosa) of the eyelid allows the easy removal of the surplus fat («transconjunctival blepharoplasty»).

This is a very common intervention that does not leave any scar on the skin and healing is usually fast. In the rare cases when surplus skin also must be removed, the incision is done under the implantation line of the eyelashes, which makes the scar rapidly very inconspicuous.

During this operation, the skin and the muscle of the lower eyelid are tightened and the fat pocket is removed.

When only the fat pocket is removed (which is the case for most patients) there is no visible scar because a little incision hidden on the inner part – mucosa - of the eyelid allows the easy removal of this fat. This procedure is called «transconjunctival blepharoplasty». In the rare cases when surplus skin also must be removed, the incision is done under the implantation line of the eyelashes, which makes the scar rapidly very inconspicuous.

WHAT KIND OF ANAESTHESIA WILL BE USED?

Most often local anaesthesia with or without infusion. It may be general.

HOW LONG WILL SURGERY TAKE?

Between 20 and 30 minutes according to the technique.

HOW LONG DO I HAVE TO STAY AT THE CLINIC?

Few hours (ambulatory intervention: admission in the morning and departure a few hours after surgery).

WHEN CAN I GO BACK TO WORK?

Six to eight days after the intervention. However, you recovery time depends on the natural healing capacity of your body.

WILL I FEEL PAIN AFTER SURGERY OF THE LOWER EYELIDS?

Normally there is no pain after this intervention.

MUST I STOP DOING SPORT?

Physical activities should be avoided for two weeks.

DO I HAVE TO STOP SMOKING BEFORE AND AFTER THE INTERVENTION?

Smoking can increase the risk of infection of the respiratory tract and can slow down the healing process. If you smoke, we will ask you to reduce or stop smoking in the weeks before and after the operation.
